The Islington Assertive Outreach Team (AOT) set out to increase Dialog+ completion rates from 31% to 60% - because service users should be involved in shaping their care plans. 

What made the difference?

• Simple visual aids
• Pre-visit forms and co-completing during appointments
• More stable staffing = more consistent practice

What we learned:
Small changes really do add up. The next big step? Portable devices to support real-time recording and keep the momentum going.
