Ensuring all service users and carers receive the right support at the right time across the Camden and Islington Memory Service
This project is to ensure that our Service Users and Carers receive the right support at the right time. This was measured and evidenced by the complexity of caseloads and the number of Service Users who were stepped up or down based on their current levels of support needs.

'Back in October 2023 when we launched our step up step down process during our first ever joint away day, we began to capture our caseload movement across both C&I memory services. We are happy we can provide you with ongoing audits summarising all of our caseload movement between our Nurses and Assistant Practitioners/Dementia Navigators. We are now in a better position to share, highlight and evidence the value of all the hard work that you do in our memory services post-diagnostic pathway, and how we are responsive to service users according to their changing needs.' - Nicole Deenmamode
